I got SWD error both FRDM-KL25Z and Arch Max 1.1

I got SWD error from fail.txt. After I put a blinker to my mbed board. This simptom occurred in FRDM-KL25Z and Arch Max 1.1. I'm very confused.

Can you update the firmware for each?

I have quick tested the FRDM-KL25Z with latest firmware and found no problem (blinky example works fine).
